YOUR DATA MATTERS

From July 2021, there are changes to who can see your anonymous patient information.  NHS Digital will be extracting information, you can opt out of the national extraction by clicking on the link on the home page.  You can opt out of your GP data being extracted by completing the opt out form - the link to this is on the home page, or you can do this using the online consultation link.

You can choose whether your confidential patient information is used for research and planning. See our tab on the right - GDPR & Your Data for further information.

IMPORTANT PATIENT INFORMATION

HELP US TO HELP YOU

You will have noticed that our practice is becoming bigger and busier.  We have taken on many new patients over the past 3 - 4 years.  GP surgeries are not allowed to close their practice list and so we need to work out how best to deal with the increased demand.  At the moment, since Covid, we are experiencing unprecedented demand, we are trying to make contact with as many of our patients as we can in the working day.  The Doctors can talk to 2 or 3 patients by telephone in the time it takes to see 1 patient face to face and many of our patients prefer to speak to their doctor this way.  We are still seeing patients who need to be examined, your doctor will arrange a time with you if you need to come to surgery.

Please understand that we are doing our best to cope with the demand and please consider using pharmacists and self treatment as a first line.  IF YOU NEED TO SEE A DOCTOR, WE ARE HERE TO HELP.

TRIAGE

Telephony

We have updated our telephone system to deal with as many calls as we can.  We now have a call waiting system, we can have a queue of up to 6 calls on each of our lines.  If the demand is more than this, the system will ask you to try again, this is to balance waiting in the queue but preventing patients having to hold for a significant amounts of time.  

KOOTH

The practice is pleased to offer an online service to adolescents and young people needing emotional health and wellbeing support and guidance.  This is available to any young person aged between 11 and 25 years of age that lives or is registered with a GP within the Bolton area. 

The BACP accredited counsellors can support young people with a wide range of issues, from low mood, family or friendship problems, to issues around gender identity, self-harm, eating disorders, anxiety and depression. The service is suitable for young people awaiting a referral or intervention from CAMHS, or for those who would not meet CAMHS criteria, who may just need support, advice and guidance

NEW SALARIED GPs

We are very happy to welcome, Dr Arif Chhabu and Dr Jasmine Close, who will be joining the practice this month.  Many of you may have met them already as they completed their GP training with us.

The Oaks Family Practice has a long and successful history as a training practice.  W currently have 5 Trainee GP Doctors with us, many of our patients will know that trainee GP's are qualified doctors who are completing their training to become GP's.  We are happy to welcome

FRIENDS AND FAMILY RESPONSES

Thank you to everyone who has taken the time to complete a Friends and Family response at the surgery, these are very encouraging and most of our patients seem to be very happy with the service we provide.  You can also complete these online by clicking the link below.  Please help us to improve our service by replying to the link we will send to your email, or by filling in one of these quick surveys in practice, and let us know what you think of your practice. 

ALLOCATED GP

All of our patients now have an allocated GP who has overall responsibility for their care.  This does not affect your choice as to which GP you book an appointment with and there may be times when your allocated GP is not available.  The practice remains a group practice and you are able to see any of the Doctors.  Please contact reception where we will be happy to inform you of your allocated GP.  If you wish to change your allocated GP this can also be arranged.  Please note that your preferred GP may not be in surgery on the day you wish to speak to them, the receptionist will inform you of this and an alternative Doctor will be available or you can speak to your preferred GP on the next day they are in surgery.

ORDERING REPEAT PRESCRIPTIONS ELECTRONICALLY

The practice is now able to offer the Electronic Prescription Service, please see the tab for more information. Please register for a PATIENT ACCESS account using the link on the Prescription tab.  You can also order prescriptions using the MyGP App on your mobile phone.  This can be downloaded using your app provider.

Are you Due a Review? COPD, Asthma, Diabetes.....

We are changing the way in which we recall our patients for longterm condition reviews.  To help prevent you being invited to separate appointments for different conditions, we have developed a system which will invite you yearly, in your month of birth, to attend the surgery for a full review of all your nurse led conditions.  Hopefully this will mean less unnecessary recalls for you and a regular yearly check up at a time of year that you will expect.

Patient Participation Group (Patient Forum)

Our PPG meet regularly  to continue their valuable work. We were pleased to unveil our recent patient satisfaction survey results. Please see the link on the right hand side of this website to find out about joining the group or ask staff for more information. New members are very welcome.

IMPORTANT: Please Update Contact Details 

The practice have had a couple of incidents recently where GPs have had needed to get in touch with patients about results (in some cases as a matter of urgency) and contact details (especially phone numbers) have been inaccurate. Please ensure we have the correct details for you. You can complete our quick form below. This is for your benefit as well as ours.

 

Texting Service

We offer a texting service to confirm, and remind you about your appointments, this also gives you the option to cancel any booked appointments. We have texted all our patients with mobile numbers offering them the chance to opt out of this service.  If you do wish to opt out at any time, please text back the message 'OPT OUT' and we will arrange this for you.  If you wish to cancel an appointment following the text reminder, please text back 'CANCEL' .  We may also text patients to offer particular services or vaccinations.

If you are not receiving the text message reminders, please contact the surgery to confirm your mobile number.
